What Is It?
This form will be used to collect information on technology related projects or purchase requests of equipment or software to support curriculum delivery. Please include here any projects (i.e. building of a lab, upgrading equipment, etc) that will require significant time, or any purchases that will be needed for the upcoming academic year.
This request must be made by a Department Chair who has access to their departmental budget. If you have multiple requests, please fill out the form for each request.
